Etravirine

Etravirine is primarily used in combination therapy for treating HIV-1 infection in patients who have previously been treated with other antiretroviral drugs. Its unique mechanism allows it to be effective even when other treatments have failed due to resistance mutations. It is indicated for both adult and pediatric patients aged two years and older who are treatment-experienced . Furthermore, etravirine is often combined with other antiretroviral agents to enhance therapeutic efficacy and minimize resistance development.

Etravirine exhibits potent antiviral activity against HIV-1 by noncompetitively inhibiting the reverse transcriptase enzyme. This inhibition occurs through binding to a hydrophobic pocket near the active site of the enzyme, inducing conformational changes that prevent DNA polymerization. Etravirine's unique structure allows it to maintain efficacy against a wide range of HIV-1 variants, including those with mutations that confer resistance to earlier NNRTIs such as efavirenz and nevirapine . Its high genetic barrier to resistance makes it a valuable option for treatment-experienced patients .
