Betahistine is primarily indicated for:

• Ménière's Disease: It alleviates vertigo symptoms by improving inner ear blood flow and reducing fluid pressure.
• Vestibular Disorders: It is also used off-label for other balance-related disorders due to its effects on neurotransmitter modulation.

Despite its efficacy in Europe and other regions, it was withdrawn from the U.S. market due to insufficient evidence supporting its effectiveness.

The biological activity of betahistine is multifaceted:

• Histamine H1 Receptor Agonism: This action leads to vasodilation and increased permeability of blood vessels in the inner ear, reducing endolymphatic pressure.
• Histamine H3 Receptor Antagonism: This effect enhances the release of neurotransmitters such as serotonin and acetylcholine, further contributing to its therapeutic effects in managing vertigo and balance disorders.
• Neurotransmitter Modulation: By increasing levels of neurotransmitters like norepinephrine and gamma-aminobutyric acid (GABA), betahistine helps stabilize vestibular function.
