2,3-Dimercaptopropan-1-Ol

Dimercaprol is primarily indicated for:

• Acute poisoning: Effective against arsenic, mercury, gold, and lead poisoning.
• Wilson's disease: Used as a copper chelator in severe cases.
• Chemical warfare antidote: Specifically against Lewisite exposure.

It is often administered in conjunction with other chelating agents like edetate calcium disodium for enhanced efficacy against lead poisoning.

Dimercaprol exhibits significant biological activity as a chelator. It competes with thiol groups on metabolic enzymes for binding with heavy metals, thus reversing enzyme inhibition caused by metal toxicity. While effective in mobilizing metals from tissues, it can also inadvertently increase metal concentrations in certain organs, leading to potential toxicity. For instance, although it enhances cadmium excretion, it may also elevate renal cadmium levels.

Adverse reactions include hypertension, tachycardia, nausea, vomiting, and localized pain at the injection site. Severe reactions may involve nephrotoxicity and neurological effects such as seizures.
