Altretamine is primarily utilized in oncology for treating ovarian cancer. It has been shown to be effective in cases where traditional alkylating agents fail. The drug is administered orally in capsule form, typically taken four times daily over a cycle of 14 or 21 days, with potential adjustments based on patient response and side effects.

Altretamine exhibits cytotoxic effects primarily through DNA modification and cross-linking. This action inhibits DNA replication and transcription, which are critical for cell division and survival. Although in vitro studies have shown negative results for direct alkylating activity, the drug's metabolites are thought to play a significant role in its therapeutic efficacy against resistant ovarian tumors.
