Milnacipran's primary application is in the treatment of fibromyalgia. It helps alleviate symptoms such as chronic pain and fatigue by balancing neurotransmitter levels in the brain. Additionally, its potential use in treating major depressive disorder has been explored, although it remains unapproved for this indication in the United States. Ongoing research continues to investigate other possible therapeutic applications of milnacipran based on its pharmacological profile.

Milnacipran exhibits significant biological activity as a dual reuptake inhibitor for serotonin and norepinephrine. Its action on serotonin and norepinephrine transporters occurs in an approximate ratio of 2:1, favoring norepinephrine. This mechanism contributes to its efficacy in managing fibromyalgia symptoms by modulating neurotransmitter levels that influence pain perception and mood regulation. Additionally, milnacipran does not significantly interact with various receptors such as alpha-1 adrenergic, dopamine D1 and D2, or muscarinic acetylcholine receptors, which may reduce the likelihood of certain side effects commonly associated with other antidepressants.
